Medicaid Aged and Disabled Waiver

The Medicaid Aged and Disabled Waiver (ADW) Program provides in-home and community services to individuals 18 years of age and older who are medically and financially eligible. Medical eligibility is based on a functional assessment by a medical professional. (A Medical Necessity Evaluation Request must be completed by applicant and the applicant's physician.) Financial eligibility is determined at the county Department of Health and Human Resources offices. Information on eligibility can be found on the Bureau for Medical Services website. Policies are described in the Bureau for Medical Services’ Aged and Disabled Waiver Manual.


To apply to become a provider for the Medicaid Aged and Disabled Waiver program:

  1. Review the Medicaid Program Manual chapters 100, 200, 300, 400, 501, 600, 700, and 800 located on the West Virginia Bureau for Medical Services website.
  2. Review the Site Monitoring Tool (updated 4/1/21) available on this website.
  3. Complete the Certification Application (updated 6/2/23) and mail to:
         Director of Medicaid Operations

         West Virginia Bureau of Senior Services
         1900 Kanawha Blvd., East
         Charleston, WV 25305-0160
  4. Once the application is received by the Bureau of Senior Services, a representative will contact you to schedule an onsite review.
  5. If you meet all certification requirements based on the onsite review, the Bureau of Senior Services will notify WV Medicaid Payor.
  6. WV Medicaid Payor will send you a Medicaid Aged and Disabled Waiver Enrollment Application.
  7. The Enrollment Application must be returned to WV Medicaid Payor.
  8. Once WV Medicaid Payor determines the Enrollment Application is complete, they will notify you and provide you with a Medicaid provider number.

Services provided in the ADW include:

  • Case Management – Development of a service and support plan by a case management agency that reflects the wishes and preferences of the ADW member. View Case Management agencies and Case Management Selection forms.    
  • Personal Attendant – Long-term direct care and support services (assistance with personal hygiene, nutritional support, and environmental maintenance) that are necessary in order to enable an individual to remain at home rather than enter a long-term care facility.   • Agency Transfer – If you wish to transfer to another Case Management or Personal Attendant agency, complete this form and submit to the Bureau of Senior Services Transfer Coordinator. Download Request to Transfer form here.  
  • Transportation – an ADW member may be transported by the homemaker in order to gain access to services and activities in the community 
  • RN Assessment and Review – a registered nurse will complete assessments of the ADW member at regular intervals to ensure that the member’s plan of care is meeting his/her needs    
  • Personal Options – ADW members have a monthly budget that they use to hire caregivers/employees and purchase needed ADW services to assist with activities of daily living. View Overview of Personal Options.

Quality Assurance and Improvement Advisory Council

The Aged and Disabled Quality Assurance and Improvement Advisory Council consists of fifteen members who are former or current Aged and Disabled Waiver members (or their legal representatives), providers, direct care workers, family members, advocates, or other individuals interested in quality home and community-based services.

 

The members meet quarterly from 10:00 a.m. to 12:00 p.m. Public Comment period is available at the end of each meeting. Meetings are held virtually at this time.
