The Medicaid Personal Care Program includes hands-on, in-home services provided to those who are medically and financially eligible. A medical assessment is completed by a physician. For financial eligibility information, please contact your local Department of Health and Human Resources office or visit their website. Services are provided by Personal Care agencies who must have an approved certificate of need. Personal Care Selection forms can be found here. Policies are described in the Bureau for Medical Services’ Personal Care Manual.
