Medicaid Personal Care

The Medicaid Personal Care Program includes hands-on, in-home services provided to those who are medically and financially eligible. Medical eligibility is based on a functional assessment by a medical professional. (A Personal Care Medical Necessity Evaluation Request must be completed by the applicant and the applicant's physician, physician's assistant or nurse practitioner.) For financial eligibility information, please contact your local Department of Health and Human Resources office or visit their website. Services are provided by Personal Care agencies who must have an approved certificate of need. Services provided in Personal Care include assistance with:

  • Personal Hygiene
  • Dressing
  • Feeding
  • Nutrition
  • Environmental Support (Light Housekeeping)
  • Health-Related Tasks
