Before applying for your Golden Mountaineer Discount Card, please note the following:
- You must be a resident of West Virginia and 60 years of age or older.
- If you have a valid West Virginia driver's license or photo ID, you should automatically
receive a card in the month after you turn age 60.
- If you have a card, please do not apply for another one.
- The Golden Mountaineer Discount Card is bright yellow and has "Caremark" printed on the back of it.
